Please Complete the following items before your first visit to our office.
1. Download a New Patient Packet and fill out and print all pages before your visit.
2. Gather all medications, and bring them with you to your appointment.
3. Gather all cardiac related records and bring them with you.
4. Bring a list of any known allergies that you have.
